This is Part 1 of a two-part series about the approach to pediatric ECGs. This series was developed by Eric King, a medical student at the University of Alberta. This podcast was developed with the help of pediatric hospitalist and medical educator Dr. Karen Forbes, pediatric cardiologist Dr. Joseph Atallah and the help of the PedsCases team. An Electrocardiogram (EKG) records the electrical activity of the heart. It tells how fast the heart is beating, if it’s the right rhythm, and many other thi Unfortunately, until faced with a patient with an arrhythmia or structural heart disease, pediatric practitioners generally receive limited exposure to ECGs.

The paediatric ECG is both a friend and foe to the general paediatrician. It can contain a wealth of direct information, but for most, creates an aura of uncertainty. I became interested in ECG interpretation as a junior registrar when I, on a post take ward round, was told to ‘send it across to cardiology for interpretation’. ECG’s can already present a significant challenge, however there is a fear associated with the paeds ECG. There should not be, as the same principles that apply to the adult ECG and the use of the ‘ECG in 20 Seconds Approach’, also apply to children.

The closer proximity of the ECG leads to the heart muscle can exaggerate the voltages. V2 through V5 are the most likely to be artificially exaggerated. The electrocardiogram remains an important test for the diagnosis and evolution of congenital heart disease, arrhythmias or other heart conditions during childhood. The basic principles of pediatric electrocardiogram are identical to those in adults, but in children, the EKG presents different features, conditioned by the patient's age.

The ECG must always be interpreted using a systematic approach in order to minimize the probability of missing significant abnormalities. The following parameters must be assessed, in chronological order: 2021-3-10 · Pediatric ECG With Junctional Rhythm Tue, 10/07/2014 - 00:07-- Dawn. This ECG, taken from a nine-year-old girl, shows a regular rhythm with a narrow QRS and an unusual P wave axis.

While the electrical waveforms and basic approach to electrocardiogram (ECG) interpretation are similar for pediatric and adult patients, some important differences must be recognized to provide appropriate care for infants and children.
